The enrollment form is not currently available. If you did not submit the waiver or enrollment form by the deadline and you are eligible for the plan you have been automatically enrolled in the student health insurance plan.

Graduate students at Lasell University are not eligible for the Student Health Insurance Plan (SHIP) offered by the University. The Lasell University eligibility policy complies with the Massachusetts State Regulations. It is important to note the following:
